Online Application for Certification of a Vital Record — How it Works

Step 1) Fill-in the Online Form

Depending on the Certificate Type you must provide information such as:

The name(s) on the Certificate
The names of the Parents
The Date of the Event
The Location of the Event
Your Name, Relationship, and Contact Info
Your Reason for the Request

Step 2) Upload & eSign

Upload the required ID Documentation & then Electronically Sign your Application

Please note: When requesting an amendment to a Vital Record, hard copies of your signed application and supporting documents must be delivered to the Office of Vital Records.

Pay online with a Credit Card to complete the submittal process.

OVR assigns a Tracking ID to each application. Use this ID to:

The Office of Vital Records is currently processing online applications submitted on 12/03/2025. Please allow 20 days for mail delivery before you contact our Call Center. If this timeline does not meet your needs, we offer expedited processing through VitalChek, or walk-in service with same-day processing can be found at these locations.
