Online Application for Certification of a Vital Record — How it Works

Step 1) Fill-in the Online Form
Depending on the Certificate Type you must provide information such as:
  • The name(s) on the Certificate
  • The names of the Parents
  • The Date of the Event
  • The Location of the Event
  • Your Name, Relationship, and Contact Info
  • Your Reason for the Request

Step 2) Upload & eSign
Upload the required ID Documentation & then Electronically Sign your Application
Please note: When requesting an amendment to a Vital Record, hard copies of your signed application and supporting documents must be delivered to the Office of Vital Records.
Pay Online
Step 3) Pay Online & Submit
Pay online with a Credit Card to complete the submittal process.
Track your Application OVR assigns a Tracking ID to each application. Use this ID to:

The Office of Vital Records is currently processing applications received in the office on 11/14/2024 with a current estimated processing time of 2 week. Please allow 20 days for mail delivery before you contact our Call Center. If this timeline does not meet your needs, we offer expedited processing through VitalChek, or walk-in service with same-day processing can be found at these locations.
